How to set up DHCP server on Ubuntu

A new Kubuntu 12.04 install is not getting IP addresses from either the wired connection nor from any wireless connection.

Stock install - everything seems to work, until you reboot. Then you get a 2-minute timeout and no network until you run sudo dhclient.
